Testicles ache and swelled slightly?

The only sexual encounter I may have had was when I was 12 and was playing with my dogs(they licked my hand) and then went to masturbate without washing my hands. A few months later a black spec, which was quite solid and hurt, was on the underside of my shaft at the midpoint. I tore it off, which hurt so much. I didn't have any problems since until 3-4 later, which was just random occasions of eerie ball ache when riding the bus home. Now it's another 3 years later and the left section of my ball sac is just slightly larger; it's like a liquid is occupying it with a little bit of slippery solid,which doesn't seem to be rock solid. I also get an eerie ball ache from now and then. Also, I don't have access to a doctor, I've been asking my parents to take me to a doctor for about 3 months now. I'm 18 now.
